The Program
The Integrated Team Care (ITC) team works to assist Aboriginal and Torres Strait Islander people to obtain primary health care in the allocated region. Its aim is to coordinate services and improved health care for clients with chronic conditions who require a multidisciplinary health care approach.
The Role
As the ITC Coordinator you will:
- Contribute to improving health outcomes through multidisciplinary care
- Support closing the gap initiatives by improving access to culturally appropriate care services.
- Work with individuals, their families and healthcare providers to support the management of chronic conditions.
- Building strong relationships with community and stakeholders to ensure appropriate and effective care
- Support and coordinate ITC Team members to achieve the goals of the program.
About You
We’re looking for someone who is passionate about supporting Aboriginal and Torres Strait Islander communities, has strong communication and advocacy skills and a deep understanding of the needs of Aboriginal and Torres Strait Islander people.
You bring a strong commitment to confidentiality and culturally safe practice, along with an understanding of working in a multidisciplinary team to ensure integrated health care.
