How our Care Finders can help you;

Care Finder is aimed at persons 65 years and over (50 years for Aboriginal and Torres Strait Islanders) who don’t have a support person to help them and are having difficulty accessing My Aged Care and other community supports.
It is also available to persons 50 years and over (45 years for Aboriginal and Torres Strait Islanders) on low income, homeless, or at risk of being homeless.

Care Finder is available to older persons who do not have a support person to help them and are finding it difficult to access My Aged Care and other community supports.
This may be because they;

  • Don’t know what services are available or how to find them

Our Care Finder Team is here to help you;

  • Completing forms and understanding aged care service agreements.
  • Connecting to other support in the community, such as health and fitness, mental health, housing, and community groups that meet your individual needs.
