How YCCC can help you;
The Youth Triple C program is aimed at coordinating and better-integrating care for an individual experiencing severe and persistent mental illness across multiple providers in order to improve outcomes.

Primarily, consultation and liaison may occur with primary healthcare providers, acute health, emergency services, rehabilitation and support services, family, friends, other support people and carers and/or other agencies that have some level of responsibility for the client’s treatment and/or well-being.

The YCCC Clinician will Support young people to effectively manage their symptoms and avoid unnecessary hospitalisation
How do I access this service?
You can access YCCC services by completing an online self-referral via the link below. Alternatively, a referral can be made by family members or friends, GP, Counsellor or other service providers

This is a FREE service, made possible by funding and support from Country SA PHN (CSAPHN)
