Specialised, no-cost service aimed at improving access to wrap-around coordinated care and clinical services for youths aged 12-25 who are at risk of or currently experiencing severe or complex mental health concerns
To assist youths currently struggling with severe or complex mental health concerns the YCCC can:
- Deliver evidence-based interventions and psychological interventions (Cognitive Behavioural therapy)
- Promote better management of the physical and sexual health of youths.
- Encourage collaborative practice amongst service providers to benefit youths within the region.
- Ensure pathways are in place to enable and support clients to transition seamlessly between services as their needs change.
- Support clients to effectively manage their symptoms and avoid hospitalisation.

You can access YCCC services by completing an online self-referral via the link below. Alternatively a referral can be made by family members or friends, GP, Counsellor or other service providers
