How our care finders can help you;

Care finder is aimed at persons 65 years and over (50 years for Aboriginal and Torres Strait Islanders) who don’t have a support person to help them and are having difficulty accessing My Aged Care and other community supports.
It is also available to persons 50 years and over (45 years for Aboriginal and Torres Strait Islanders) on low income, homeless, or at risk of being homeless.

 

Care finder is available to older persons who do not have a support person to help them and are finding it difficult to access My Aged Care and other community supports.
This may be because they;

  • Have trouble reading and writing in English
  • Find it difficult to understand information and make decisions
  • Have worries about engaging with Aged Care or Government services
  • Don’t know what services are available or how to find them

 

Our care finder Team is here to help you;

  • Connect with My Aged Care help you through the assessment process.
  • Completing forms and understanding aged care service agreements.
  • Connecting to other support in the community, such as health and fitness, mental health, housing, and community groups that meet your individual needs.
  • Our team will also check in with you every now and then to make sure the services you are connected to are meeting your needs.

This is a FREE service, made possible by funding and support from Country SA PHN (CSAPHN)

Country & Outback Health offer the care finder service across South Australia, partnering with health organisations to extend the service where it is needed most.
