• A parent/legal guardian may need to give consent for you to access a particular service (if you are under 16).

• Inform that Duty of Care is detailed within the Welcome Pack.
• We collect personal information about you that is helpful for us to provide services to you (Name, DOB, Contact Details, Gender & Sexual Identity, language spoken at home).
• You will be asked a few questions about your well-being before each service to track your progress. This is done via hAPI
• We will create a file to document all the care you receive.
• We can only collect this information from you if you give consent.

• Interpreter services can be arranged in advance to support you/your FAF to communicate with your worker during the session. We do have the support of a translating iPad app.
• More than one worker can support you across various programs, and you have the right to work with someone you connect with.

• Your preference (along with evidence about what works) guides decisions about your care.
• We provide your FAF opportunities to participate in your care and/or in our FAF reference group. The level of involvement and support of your FAF in your care is at your discretion.

• We appreciate all feedback (compliments, suggestions & complaints) about your care.
