Country & Outback Health is a not-for-profit organisation focused on building healthier and happier communities.  Delivering a diverse range of mental health, allied health, addiction support, Aboriginal and Torres Strait Islander health, NDIS services as well as support services for Young People within our headspace centres.

About the role:

  • Part-time Monday to Friday (52.5 hours per fortnight)
  • Located in Whyalla

We are looking for compassionate and dedicated professionals to provide specialised support, helping people navigate the complexities of aged care and connect with essential community services through our Care Finder Program.

As a Care Finder Specialist, you will play a key role in:

  • Guiding individuals through the process of understanding and accessing aged care supports.
  • Connecting people with relevant community services and supports tailored to their needs.
  • Improving outcomes for our clients by enhancing coordination between health, aged care, and other local systems.
  • Bridging gaps in understanding and accessibility, ensuring seamless integration of services at the local level.

About You:

  • Qualification in Social Work, Health, Aged Care, Human Services or relevant equivalent work experience.
  • Ability to establish and maintain effective relationships with clients and the community
  • Knowledge of the My Aged Care process and related service providers.
